  "abstract": "Introduction The treatment of small, wide-necked intracranial aneurysms (IAs) remains a significant challenge for neurointerventionalists, driving the continuous expansion of the endovascular armamentarium. Among the latest and most promising advancements are theTarget Tetra coils (Stryker Neurovascular).Aim of study This two-centre study investigates the short-term efficacy and safety of Target Tetra coils and explores the nuances of their usage.Method A consecutive series of ruptured and unruptured IAs treated with Target Tetra coils from January 2024 to February 2025 was prospectively recruited. Primary outcomes included adequate aneurysm occlusion (Modified Raymond-Roy Classification) at the first follow-up, and device- and procedure-related morbidity and mortality. Secondary outcomes included the use of adjunct devices, immediate occlusion, packing volume (PV), retreatment rate and clinical outcomes using the modified Rankin scale. A secondary analysis investigated the influence of using Tetra-only coils versus non-Tetra-predominant coils.Results Seventy-five patients with 80 aneurysms (56 ruptured, 24 unruptured) were included. Coil embolisation was successful in all cases. At the first follow-up (mean 5 months), adequate occlusion was achieved in 94.3% (33/35) of patients. Three (3.8%) procedural-related ischaemic strokes and two device-related technical issues occurred, including one case of coil migration and one case of coil-detachment failure. Adjunct devices were required in 13 patients, and 85% achieving immediate adequate occlusion. The median PV was 30.5%. One retreatment was required. Short-term mRS 0–2 was achieved in 94.3%. All-cause mortality was 8%.Conclusion Target Tetra Coils demonstrate promising short-term safety and efficacy.
