  "abstract": "Introduction A wide-neck aneurysm of posterior communicating artery (pcoma) often requires advanced treatments such as balloon remodeling, stent-assisted coiling, and flow-diverter stents. The availability of novel intrasaccular flow-disruptor devices enables endovascular treatment (EVT) of wide-neck bifurcation intracranial aneurysms, which can also be considered in emergency situations.Abstract C122 Figure 1Case Description 78-year-old female patient (Pt.) with a small ruptured wide necked pcom aneurysm with pcoma arising from the aneurysm neck, was a candidate for endovascular treatment (EVT). Intrasaccular flow-disruptor Contour was choosen in order to preserve the origine of the pcom. Despite the challenging access from the internal carotid artery, the device was successfully deployed in the aneurysm, resulting in significant contrast media stagnation after the detachment. 2 weeks after the procedure the single antiagreggation therapy was stopped. Pt. was completely recovered. 19 days after the initial treatment the aneurysm reruptured presenting left eye ptosis (Glasgow coma scale 14). Retreatment of the small reruptured aneurysm already treated with the Contour, was performed through a wedged microcatheter, positioned between the device and the aneurysm wall, along with additional coiling, resulting in aneurysm filling beyond the previous device in place.Abstract C122 Figure 2Conclusions Contour sizing in wide-neck ruptured aneurysm can be trick. Consideration of bigger device proposed by sizing chart is option, in order to prevent endoleak.
