  "abstract": "Introduction Approximately 10% to 20% of large vessel occlusion (LVO) strokes involve tandem lesions (TLs), defined as intracranial LVO combined with stenosis or occlusion of the cervical internal carotid artery. These patients typically have worse clinical outcomes compared to those with isolated intracranial occlusions. Mechanical thrombectomy (MT) has improved prognosis in TL cases; however, the optimal strategy for managing the extracranial cervical lesion remains unclear.Aim of Study To evaluate different outcomes of carotid artery stenting vs no stenting among patients with TL-LVO in our institution.Method We conducted a retrospective observational study of patients with tandem occlusions treated at Parc Taulí Hospital between 2021 and 2025. The study aimed to compare the clinical and procedural outcomes, demographic characterisctics, vascular risk factors, baseline ASPECTS (Alberta Stroke Programme Early CT Score), NIHSS (National Institutes of Health Stroke Scale) at admission periprocedural complications, and functional outcome (mRS) at 90 days.Results A total of 39 patients were included, all of whom presented with acute ischaemic stroke due to tandem occlusion involving both the intracranial circulation and the cervical internal carotid artery (ICA). Among these, 13 patients underwent balloon angioplasty alone, while 26 patients received ICA stenting in addition to mechanical thrombectomy.Conclusion This study indicate that combining acute carotid internal artery (c-ICA) stenting with intracranial thrombectomy may be the most effective treatment strategy for patients with tandem lesions (TLs)Conflict of Interest No",
