  "abstract": "Introduction Ruptured fusiform intracranial aneurysms are rare and account for only 4.5% of all subarachnoid hemorrhages. If not treated, there is a high risk of rebleeding and mortality up to 71% and 47%, respectively. Traditional treatment methods include sacrificing of the parent artery, aneurysm wrapping or bypass. However, such techniques are often associated with a high perioperative risk. Both surgical and endovascular conventional reconstructive treatment methods can be difficult or sometimes even impossible.Abstract C67 Figure 1Case Description Four patients presented with subarachnoid hemorrhage from proximal fusiform PICA (posterior inferior cerebellar artery) aneurysms and were treated with Silk Vista Baby flow diverters. No additional coiling was performed. All patients received 30–60 mg prasugrel before the procedure and 300–500 mg iv aspirin as well as intravenous heparin during the procedure. Minimum vessel diameter was measured only 0.5 mm. There was one acute stent thrombosis immediately after the flow diverter opening. This was resolved with aspiration thrombectomy and intravenous eptifibatide bolus medication. No rebleedings were discovered. All patient recovered clinically well.Abstract C67 Figure 2Conclusions Due to location of the aneurysms and insufficient collateral circulation we evaluated that parent artery occlusion would have led to serious brain infarction and disability in all cases. Silk Vista Baby flow diverter is placed through a 017 microcatheter, which makes cathetrization of the small vessels much smoother compared to the traditional flow diverters that are placed through bigger microcatheters.
