  "abstract": "Introduction Dural arteriovenous fistulas (dAVFs) can manifest with diverse clinical features. Intracranial hypertension (ICH) is a less frequently recognized but important presentation, particularly when venous outflow is impaired. Early identification of this relationship is critical to guide effective treatment.Abstract C125 Figure 1Case Description We describe two patients with progressive ICH symptoms, including headache, papilledema, and visual impairment. Imaging revealed impaired venous drainage and digital subtraction angiography confirmed the presence of dAVFs—one at the transverse-sigmoid junction and the other near the condylar confluence. Both cases were associated with contralateral transverse sinus stenosis. Endovascular embolization resulted in complete fistula occlusion. However, post-procedural venous manometry showed persistent pressure gradients across the stenotic sinuses. These gradients were successfully treated with venous sinus stenting. Clinical symptoms resolved fully after intervention, CSF pressures normalized, and radiological follow-up showed resolution of venous congestion. No procedural complications were reported.Abstract C125 Figure 2Conclusions These cases suggest that ICH may contribute to the formation of dAVFs through altered venous flow dynamics. In patients presenting with dAVFs and features of ICH, investigation of venous sinus pressure gradients is advised. Addressing venous sinus stenosis in conjunction with embolization may enhance clinical outcomes, promoting symptom resolution and lowering the risk of dAVF recurrence.
