  "abstract": "Digital twins (DTs), advanced virtual models powered by AI, real-time data from wearable sensors, and predictive analytics, represent a groundbreaking approach to personalised healthcare. For transgender (trans) individuals, who often face systemic obstacles such as discrimination, limited access to affirming care and misgendering in medical settings, DTs offer a promising avenue for tailored, gender-affirming healthcare. These models can simulate the physiological and psychological impacts of interventions like hormone replacement therapy (HRT) or gender-affirming surgeries with high precision, aligning medical care with individual identities and needs. Drawing on Guerrero Quiñones and Puzio,1 who argue that DTs not only mirror a patient’s physical body but also shape their virtual identity, this commentary examines the ethical, practical and social dimensions of designing inclusive DTs for trans healthcare. It emphasises bioethical principles, participatory codesign and strategies to ensure equitable access and representation.",
