  "abstract": "Objectives To evaluate the predictive value of inflammation-based hematological indices—NLR (neutrophil-to-lymphocyte ratio), MLR (monocyte-to-lymphocyte ratio), PLR (platelet-to-lymphocyte ratio), SII (systemic immune-inflammation index), SIRI (systemic inflammation response index), and AISI (aggregate index of systemic inflammation)—for assessing disease activity and treatment response in systemic lupus erythematosus (SLE).Methods A retrospective observational study with 12 months of follow-up and quarterly visits, including SLE patients and healthy controls (HC) was conducted. Patients were stratified by exposure to biologic therapy: those initiating a biologic at visit 0 vs those not receiving biologics during follow-up. Demographic, clinical, and laboratory data were extracted from electronic medical records. Indices were calculated from complete blood counts.Results A total of 87 SLE patients and 90 HC were included ( table 1). At baseline, 47.1% had SLEDAI >5; 51.7% had hypocomplementemia and 55.8% were anti-dsDNA positive. Concomitant treatments included glucocorticoids (72.4%), immunosuppressants—methotrexate, azathioprine, or mycophenolate—(46%) and antimalarials (82.7%). Forty-four patients initiated biologic therapy (belimumab or anifrolumab); 13 discontinued, mainly for loss of response.All indices (NLR, MLR, PLR, SII, SIRI, AISI) were significantly higher in SLE than in HC (P<0.001). In multivariable linear models, higher C-reactive protein (CRP) was independently associated with increased values of all indices except PLR (NLR P=0.012; MLR P=0.032; SII P=0.019; SIRI P=0.002; AISI P=0.004). Higher erythrocyte sedimentation rate (ESR) correlated with elevated PLR (P=0.001), SII (P=0.002), and AISI (P=0.011). Clinical disease activity (SLEDAI) significantly influenced NLR (P=0.019), PLR (P=0.008), and SII (P=0.020): these indices rose in patients with persistently high activity and decreased over time in those with low activity. Among patients on biologics, inflammation indices did not differ between those who discontinued and those who continued therapy. Time of disease evolution and use of immunosuppressants, glucocorticoids or antimalarials did not significantly affect index values.Abstract PO:05:144 Table 1–2Conclusions SLE patients exhibit higher inflammation-based hematological indices than HC. Associations with CRP/ESR and with SLEDAI support their linkage to systemic inflammation and clinical activity. No treatment-related effect on indices was observed. These readily obtainable measures may serve as accessible biomarkers of inflammation and disease activity in SLE.",
