  "abstract": "Objectives Several factors contribute to the progression of end-stage renal disease (ESRD) in lupus nephritis (LN), with variations observed across different regions. Key factors include persistent proteinuria, elevated chronicity index, high creatinine levels at diagnosis, therapeutic failure after 12 months, and low complement levels. This study aims to investigate the risk factors associated with ESRD progression in LN in Guatemala.Methods Records of 138 patients diagnosed with LN followed from December 2008 to April 2025 were analyzed in a retrospective study. These patients are part of the ISLA cohort from Guatemala. A Cox regression analysis was performed, considering the common risk factors for progression of LN in Latin America. The factors included in the model are listed in table 1.Results In the univariate analysis of ESRD, key associated factors included diagnosis at age under 30 years [HR 2.5 (95% CI: 1.067 - 5.89, p = 0.032)], hypertension: [HR 3.28 (95% CI: 1.36 - 7.87, p = 0.006)], more than three comorbidities [HR 2.98 (95% CI: 1.27 - 6.97, p = 0.0010)], and presence of anti-dsDNA antibodies [HR 2.2 (95% CI: 1.17 - 4.14, p = 0.017)]. In the multivariate analysis, significant findings included an HR of 3.44 (95% CI: 1.14-10.39, p = 0.028) for LN diagnosed under 30 years and an HR of 2.49 (95% CI: 1-6.56) for hypertension. The Cox regression model showed a significant overall fit (X2= 1.72, p = 0.019), indicating that these variables enhance the prediction of ESRD risk in LN, with an overall HR of 19.73.Abstract PO:09:229 Table 1Predictors of progression end end-stage renal disease in patients with lupus nephritisAbstract PO:09:229 Figure 1–2Conclusions This study identifies the most significant factors associated with predictive ESRD in LN in Guatemala. These factors include an early age at diagnosis of LN, hypertension, having more than three comorbidities, and the presence of anti-dsDNA. Together with other variables included in the model, these factors contribute to the overall progression of the disease, increasing risk over time. However, this study has limitations due to its retrospective nature, as we lack information on additional risk factors that could be incorporated into the model. Nonetheless, the findings correlate with data from other populations worldwide.",
