  "abstract": "Objectives Neuropsychiatric SLE (NPSLE) represents one of the major clinical entities contributing to both mortality and morbidity. Due to the heterogeneity of the clinical spectrum, the multiple mechanisms involved in its pathophysiology, and the limitations of diagnostic tools, the diagnosis and management of NPSLE remain challenging. In this study, we aimed to evaluate the frequency and distribution of neuropsychiatric manifestations and their associations with laboratory parameters in SLE patients followed at our center.Methods A total of 277 patients who presented to our clinic within the last six years and met the 2019 ACR/EULAR classification criteria for SLE were included. Demographic, clinical, and laboratory data were evaluated retrospectively. Patients’ age, sex, antinuclear antibody (ANA) and its subpatterns, erythrocyte sedimentation rate, and CRP levels were recorded. Neuropsychiatric manifestations were analyzed in two main groups—central nervous system (CNS) and peripheral nervous system (PNS)—based on the 1999 ACR NPSLE case definitions and current literature. Statistical analyses were performed using SPSS version 25. Variables were expressed as mean, frequency, and percentage.Results Among 277 SLE patients, 142 (51.2%) had at least one neuropsychiatric manifestation and were included in the analysis. Of these, 88.7% were female (n=126) and 11.3% were male (n=16), with a mean age of 43.3 ± 13.0 years. ANA positivity was observed in 76.8% of patients ( figure 1). The most frequent CNS manifestations were headache (37.3%), mood disorders (17.6%), anxiety (16.9%), and cerebrovascular events (14.1%). Less common findings included epilepsy (5.6%), psychosis (2.1%), demyelinating syndrome (2.1%), and myelopathy (2.1%). PNS manifestations were less frequent, most commonly polyneuropathy (4.2%) and cranial neuropathy (2.8%) (table 1). The findings highlight the importance of immunologic activity in NPSLE and demonstrate that clinical classification supported by demographic and laboratory data contributes to the early identification of high-risk patients and facilitates targeted treatment strategies.",
