  "abstract": "Objectives Fatigue, a debilitating symptom, is the most common symptom reported among patients with systemic lupus erythematosus (SLE). This study aimed to characterize the clinical and biological predictors of fatigue in SLE.Methods 144 patients fulfilling the 2012 Systemic Lupus International Collaborating Clinics (SLICC) classification criteria for SLE were prospectively included. Detailed clinical and paraclinical information was systematically collected for all patients. Fatigue was assessed using the Functional Assessment of Chronic Illness Therapy–Fatigue (FACIT-Fatigue, version 4) scale. At the same time, the SLE disease activity was measured with both the Systemic Lupus Erythematosus Disease Activity Index (SLEDAI) and the Systemic Lupus Activity Measure (SLAM) scores, while organ damage by the SLICC Damage Index.For statistical analysis, a random forest model was implemented in R version 4.4.3, using a decision forest with 200 trees to identify the most important predictors of fatigue. A cut-off value for the strongest predictor was subsequently determined using ROC curve analysis. Missing values were imputed using the missForest algorithm.Results The background characteristics of the SLE patients included are summarized in table 1. Random forest analysis identified hemoglobin level as the most important predictor of significant fatigue (FACIT-Fatigue score >34), followed by SLAM score, erythrocyte sedimentation rate (ESR), SLEDAI, and SLICC index. Articular involvement, neurological manifestations, and alopecia contributed contributed however to a lesser contributions.extent .The model achieved an area under the ROC curve of 76.9% (95% CI: 65.3–88.5%).
