  "abstract": "Objectives We examined cardiovascular risk factors (CVRFs) at enrolment and during follow-up in a cohort of patients with systemic lupus erythematosus (SLE).Methods Patients with SLE enrolled in the Amsterdam Lupus Cohort during 2008-2016 were included. We assessed traditional CVRFs (hypertension, hypercholesterolemia, diabetes, chronic kidney disease (CKD), smoking, obesity, physical activity) and SLE-specific CVRFs (disease activity, prednisone use, low C3/C4, anti-dsDNA). We analysed baseline data for all patients and longitudinal data for those with baseline visit, at least one visit during years 1-3, and at least one visit during years 4-6. CVD risk stratification was assessed using the Systemic Coronary Risk Evaluations (SCORE). Longitudinal analyses comprised describing incidence of chronic CVRFs (hypertension, hypercholesterolemia, diabetes, and CKD) and comparing non-chronic CVRFs over three time points using mixed effects models with adjustment for sex and age.Results The cohort included 236 patients at baseline (90.3% female, median age 39, median disease duration 5 years). Baseline prevalence of hypertension, hypercholesterolemia, diabetes, and CKD was 34.3%, 16.1%, 8.1%, and 18.2%, respectively ( table 1). According to SCORE, 74%, 10%, and 16% were classified into low-to-moderate, high, and very high 10-year risk of fatal CV disease, respectively. The median SLEDAI-2K score was 4 (2-6), and 51.7% of patients used prednisone (table 2). Low C3/C4 and anti-dsDNA positivity were present in 58.5% and 44.1% of patients, respectively. Lupus anticoagulant and anticardiolipin positivity was 25.6% and 11.5%, respectively. Patients with APS or lupus nephritis generally had worse CVRF profiles than patients without (i.e., higher prevalence of hypertension, hypercholesterolemia, smoking). We included 148 patients for longitudinal analysis. The incidence of hypertension was 8.3% within 3 years and 17.7% within 6 years (table 1). Most non-chronic CVRFs (smoking, obesity, disease activity, prednisone use, anti-dsDNA, low C3/C4) did not improve over time, except for physical activity (table 2). The prevalence of achieving blood pressure target increased over time (table 2).Abstract PO:10:261 Table 1–2Conclusions This SLE cohort exhibits a high, persistent burden of CVRFs, particularly in patients with APS or lupus nephritis. Significant improvements in physical activity and blood pressure control were achieved over follow-up. Future work will include assessing incident cardiovascular events to fully quantify the burden.",
