  "abstract": "Objectives Pulmonary diseases (PD) in Systemic Lupus Erythematosus (SLE) have received little attention despite it being prevalent and clinically important. Interstitial lung diseases (ILD) in SLE can be challenging to diagnose, however, thoracic ultrasound (TUS) has shown promise for the detection of ILD in connective tissue diseases (CTD). The objectives of this study were in patients with SLE to evaluate the utility of TUS in: 1) using B-lines to identify ILD; 2) using B-lines to identify PD in general and other subgroups of PD (pleural diseases and airway diseases): 3) assessing pleural irregularities to identify ILD and any PD and other PD subgroups.Methods We investigated patients with SLE fulfilling EULAR/ACR 2019 classification criteria. TUS was performed using a 14-zone protocol and evaluated by two reviewers blinded to each other’s scoring and presence of PD. TUS evaluation included number of B-lines, number of zones with pleural irregularity, and number of zones with thickening of parietal pleura. The optimal threshold for TUS positive findings were defined by Youden index.Participants were diagnosed with PD, independently of the TUS, through a multidisciplinary discussion (MDD) based on a thorough investigation including pulmonary function tests and chest high-resolution computed tomography.Results Among 185 patients with SLE PD were present in 109 (59%), including ILD in 22 (12%), pleural diseases in 35 (19%), and airway diseases in 70 (38%). B-lines most effectively detected ILD and were better at identifying ILD and pleural disease than airway diseases when compared to MDD diagnoses ( table 1). By means of Youden index, a threshold of more or equal to 10 B-lines optimally identified ILD, and correctly classifying 14 out of 22 cases. Of the remaining 8, 4 presented subtypes of ILD that we did not expect to find by B-lines (Cystic ILD and Organizing Pneumonitis), whereas 2 had a body mass index > 30.
