  "abstract": "Objectives Systemic lupus erythematosus (SLE) is a chronic autoimmune disease with heterogeneous clinical manifestations, with treatment strategies largely determined by disease activity and organ involvement. Detailed disease profiling and accurate assessment of disease activity are crucial for tailoring treatment to each patient.Methods This retrospective cohort study included 187 adult SLE patients diagnosed and treated at the Clinic of Allergy and Immunology, University Clinical Center of Serbia, and the Institute of Rheumatology, Belgrade, between 2020 and 2025. Disease activity was assessed using the SLEDAI-2K, SLE-DAS, and PGA scores, while fatigue was evaluated using the FACIT-Fatigue questionnaire. Remission was defined as a total SLEDAI-2K score of 0; lupus low disease activity state (LLDAS) as a total SLEDAI-2K score < 4 with no activity in major organ systems and PGA <1; and active disease as a clinical SLEDAI-2K score >4.Results Detailed baseline characteristics, including demographics, clinical manifestations, immunological profile, and treatments, are presented in table 1. The cohort was predominantly female (88.2%), with amedian age of 46 years (IQR 34–55) and a median disease duration of 5 years (IQR 2–12). Most patients were either in LLADS or remission, while only 58 (31.0%) exhibited active disease at inclusion, and 11 (7.0%) were newly diagnosed. Sjögren’s disease and APS were the most common associated systemic autoimmune diseases, observed in 34.1% and 11.2% of patients, respectively. The most common clinical manifestations were alopecia (35.1%), arthritis (32.4%), rash (29.2%), and leukopenia (15.7%). Anti-dsDNA and anti-Sm antibodies were positive in 35.2% and 21.8% of patients, while ENA positivity was found in 58.8%. Laboratory screening for APS detected anticardiolipin antibodies in 8 patients (5.1%) for IgM and 13 patients (8.1%) for IgG. Overall disease activity was moderate (total SLEDAI-2K 4 [2–10], clinical SLEDAI-2K 2.5 [0–13], SLE-DAS 5.96 [1.32–12.86], PGA 0.66 [0.15–1.26]), with median FACIT-Fatigue 32 (18–43). Corticosteroids and hydroxychloroquine were most frequently used (85.5% and 81.7%), followed by azathioprine (27.2%) and mycophenolate mofetil (17.3%).Abstract PO:07:189 Table 1Conclusions This study provides a characterization of the clinical and immunological features of SLE patients in Serbia, highlighting moderate disease activity and prevalent use of corticosteroids and hydroxychloroquine.",
