  "abstract": "Objectives To investigate clinical factors associated with atherosclerosis progression in Systemic Lupus Erythematosus (SLE)Methods Sixty-nine patients with SLE underwent ultrasound scans of the carotid and common femoral arterial bifurcations on two occasions (mean 63 months apart). Changes in number of plaque and in the Total Plaque Area (TPA) were assessed. We recorded for each patient the demographic characteristics, the cumulative and highest daily steroid doses taken between both scans, the other concomitant medications and the percentage of time spent in low disease activity and remission between both scans. We also assessed their cardio-vascular (CV) traditional risk factors and concluded to their CV risk category according to the 2021 European Society of Cardiology (ESC) guidelines. Finally, we counted for each patient the number of CV risk target they have achieved before the second scan.Results Thirty-eight patients had no plaque at both scans. We have not recorded any plaque regression. Among the 31patients with plaque at the second scan, 18 had increased plaque numbers while 13 had stable plaque. In terms of TPA, we haven’t recorded any stabilization. All 31 patients had increased TPA and these patients were subdivided into two groups with change in TPA above or below the median (n= 16 and 15 respectiveley). Factors associated with both increased plaque number and increased TPA were age at baseline, positive lupus anticoagulant, negative anti-La and failure to attain at least three CVD risk targets by the time of the second scan. We found no associations with disease activity or medication.Conclusions In this ethnically diverse (40% non-Caucasian) population, we confirmed earlier findings that better control of CVD risk targets reduces progression of atherosclerotic plaque. Anti-La positivity was associated with less plaque progression, which was unexpected. In this study, scanning femoral as well as carotid arteries added value but measuring TPA did not.",
