  "abstract": "Objectives Cutaneous lupus erythematosus (CLE) is an autoimmune disease that primarily manifests in the skin and mucosal tissues. Epidemiologic data on CLE remain limited. This multicenter, cross-sectional, observational study in the US evaluated the real-world distribution of CLE severity using the Cutaneous Lupus Activity Investigator’s Global Assessment-Revised (CLA-IGA-R) and CLE Disease Area and Severity Index (CLASI).Methods Adults with clinically confirmed CLE were consecutively enrolled from four US dermatology and rheumatology clinics (August 2023–April 2024). Demographics, clinical history, and disease severity data were collected during a single study visit and/or from medical records.Results The 122 participants had a mean age of 50.5 years, and most were female and Black ( table 1). Mean time since CLE diagnosis was 13.9 years, and most had a skin biopsy-confirmed diagnosis. The vast majority (93.4%) had chronic CLE and 19.7% had subacute CLE; 32.0% also had acute CLE, while 68.9% had concurrent systemic lupus erythematosus. The most commonly reported comorbidities were renal disease (26.2%), depression (20.5%), and anxiety (13.9%). Participants’ current CLE treatments included antimalarials (69.7%), topical corticosteroids (50.8%), and immunosuppressants (37.7%). The majority were overweight or obese; most had never smoked.Using CLA-IGA-R, 64% and 71% of participants were clear/almost clear for Erythema and Other Morphologic Characteristics (OMC), respectively (table 2). Fewer participants had moderate/severe Erythema (15.6%) or OMC (8.2%). The ‘no activity’ and ‘mild’ CLASI-Activity (CLASI-A) score categories each comprised ~40% of participants, while 18% were moderate/severe. The most common locations for disease activity lesions according to CLASI-A were the scalp, arms, and ears; damage was most frequent on the scalp, ears, and rest of the face.Abstract PO:07:195 Table 1–2Conclusions This study is the first to examine the characteristics and distribution of patients with CLE by disease severity using CLA-IGA-R and CLASI in a real-world setting. The large proportion of Black participants provides an opportunity to study CLE in this underrepresented population with high burden of disease.
