  "abstract": "Objectives Remission and lupus low disease activity state (LLDAS) are protective of several outcomes, including damage, mortality, hospitalizations and costs. However, these states do not include the patient’s perspective; furthermore, their association with patient-reported outcomes (PROs) have been less extensively evaluated.Methods Systemic lupus erythematosus patients from a single-center prevalent cohort were included. Three mutually exclusive states were defined: Remission as a SLEDAI-2K (excluding serology)=0, physician global assessment<0.5, prednisone daily dose<=5mg/d, immunosuppressive drugs on maintenance dose; LLDAS as a SLEDAI-2K<=4 with no new features of disease activity and no activity on a major organ, physician global assessment<=1, prednisone daily dose<=7.5mg/d, immunosuppressive drugs on maintenance dose; all other visits were classified as active. Health-related quality of life (HRQoL) was ascertained with the LupusQoL, fatigue with the FACIT-F, work productivity and activity impairment with the work productivity and activity impairment (WPAI) questionnaire, patient-reported disease activity with the Lupus Foundation of America Rapid Evaluation of Activity in Lupus (LFA-REAL PRO). For the LupusQoL and the FACIT-F the higher score, the better the patient feels, and for the LFA-REAL PRO and WPAI the lower score, the better the patient feels. Generalized estimating equations were performed using the eight domains of the LupusQoL, the FACIT-F, the work productivity impairment and activity impairment of the WPAI and the LFA-REAL PRO; the multivariable model was adjusted for age at diagnosis, disease duration, antimalarial use, gender, socioeconomic status, SLICC/ACR damage index, fibromyalgia and the Charlson comorbidity index.Results A total of 505 patients and 2690 visits were included. At 1534 (57%) visits the patients were in remission, and at 285 (10.6%) in LLDAS (but not remission); at 871 (32.4%) visits the disease was active. Remission was associated with better outcomes in all the scores, and LLDAS was associated with better HRQoL in the pain domain, and lower work productivity impairment compared to active disease ( table 1).Abstract PO:05:125 Table 1Impact of remission and LLDAS on several PROs.
