  "abstract": "Objectives The main objective is to highlight the association between SLE and uncommon and rare complications such as Sweet Syndrome, venous thrombophlebitis, and cutaneous necrosis.Methods We report the case of a 40-year-old female with a 10-year history of SLE, initially presenting with high fever, arthralgia, cutaneous rash, and serological evidence of autoimmunity (ANA in high titer and positive ENA, low C3 and C4 levels). She was treated with hydroxychloroquine (HCQ) 400 mg daily and a low dose of glucocorticoid for several years, and later HCQ was reduced to 200 mg daily. One month ago patient presented at the Rheumatology clinic with generalized rash, pain, and swelling in the left leg, profuse sweating, arthralgia, pain in the right hand’s second and third phalanges, and generalized weakness. Cuaneous infarcts and necrosis in two phalanges are seen too.Results Results Laboratory findings revealed ANA 1280, positive ENA for Ro-SS-A and La-SSB in high titer, leukocytosis with neutrophilia, lymphopenia, and slightly increased LDH. Skin biopsy was performed with typical changes of Sweet Syndrome. Doppler ultrasonography demonstrated thrombophlebitis of the main trunk of the left great saphenous vein and insufficiency in both great saphenous veins with varicosities.Abstract PO:04:111 Figure 1Conclusions This case underlines that SLE is an autoimmune disease, which can be complicated and associated with rare dermatologic and vascular manifestations such as Sweet Syndrome and venous thrombosis. Rheumatologists should maintain a high index of suspicion for such atypical manifestations to ensure timely diagnosis and management, to prevent disability and lower mortality.",
