  "abstract": "Objectives To assess the prevalence and 5-year incidence of autoimmune diseases (ADs) in Swedish cohort of patients with antiphospholipid syndrome (APS) compared with matched population controls with a particular focus on the influence of a high-risk antiphospholipid antibody (aPL) profile, categorized as triple aPL positivity.Methods This retrospective, registry-based cohort study included 250 APS patients diagnosed at Karolinska University Hospital (2014-2020), according to the revised Miyakis et al. (2006) criteria. The subgroup with triple aPL positivity was identified accordingly. Baseline prevalence and 5-year incidence of ADs were assessed. Population comparators, matched 5:1 by birth year, sex, and residence, were identified through Swedish National Patient Register and Total Population Register. ADs were classified as rheumatic (RADs) or non-rheumatic (NRADs) based on ICD-10 codes. Logistic regression estimated adjusted odds ratios (aORs) for baseline ADs. Poisson regression was used for incidence rate ratios (IRR).Results The cohort included 250 patients had a median age of 44.5 years (IQR 35–59); 68% were women. At baseline, 127 patients (51%) had one or more ADs. Among them, 96 (38%) had RAD and 55 (22%) had NRAD. Systemic lupus erythematosus was the most common RAD (30%), whereas autoimmune thyroid disease was the most prevalent among NRADs (10%). High risk aPL-profile was observed in 48% of patients. Adjusted for age and sex, triple positivity was associated with an increased odds of having a coexisting AD (aOR = 1.6; 95% CI: 1.0–2.6) ( figure 1). During follow-up, APS patients demonstrated a significantly higher incidence of newly diagnosed ADs compared to controls. The incidence rate IR was 8 per 1,000 person-years.
