  "abstract": "Objectives To determine how often autoantibodies specific for autoimmune diabetes mellitus in adults are found in SLE patients.Methods The study included 51 women with SLE without diabetes mellitus or fasting hyperglycemia. The median age of the patients was 33 [26;35] years, and the disease duration was 72 [3;156] months. SLE activity was high and very high (SLEDAI-2K >10) in 17 (33.3%) patients, and moderate (SLEDAI-2K = 5-10) in 15 (29.4%) patients. Glucocorticoids were received by 41 (80.4%), hydroxychloroquine - 38 (74.5%), immunosuppressants – 18 (35.3%) patients. Over the last year, patients did not receive biological agents. The levels of Zinc Transporter 8 (ZnT8 Ab) and Glutamic Acid Decarboxylase autoantibodies (GAD Ab) were measured using ELISA kit from RSR ltd.Results Diabetes mellitus - associated antibodies (DMa Ab) were detected in 9 (17.6%) patients with SLE: only ZnT8 Ab in 4 (7.8%), only GAD Ab in 4 (7.8%) women, and ZnT8 Ab + GAD Ab in 1 (2%) patient. There was a very weak negative correlation between SLEDAI-2K and GAD Ab levels (r= -0.29, p=0.04), but not with ZnT8 Ab (r= -0.14, p=0.92). Women with DMa Ab were less likely to have skin lesions compared to patients without DMa Ab (11.1% vs. 54.8%, p = 0.044), and their C3 complement levels were higher (0.96 [0.77;1.05] g/L vs. 0.70 [0.52;0.97] g/L, p = 0.045). However, they were also more likely to receive immunosuppressants (66.6% vs. 26.2%, p=0.051). The frequencies of other clinical manifestations (alopecia, mucosal ulcers, arthritis, serositis, nephritis, damage to the nervous system, leukopenia, thrombocytopenia, and hemolytic anemia), anti-dsDNA and C4 complement levels were comparable. Eight of the 9 patients with DMa Ab were followed prospectively for 8 to 42 months (median 28 [15;37] months) after organ-specific autoantibodies was detected, none of them developed diabetes mellitus during this period.Conclusions DMa Ab can be found quite often in SLE without DM, but its clinical significance is not yet clear. It may be related to a specific subtype of rheumatic disease or the use of immunosuppressants. Further research is needed to clarify this finding.",
