  "abstract": "Introduction Dermatomyositis (DM) and antisynthetase syndrome (ASyS) are idiopathic inflammatory myopathies (IIM) with an autoimmune pathogenesis, where microvascular abnormalities play a central role. Such changes can be detected through skin and muscle biopsies as well as typical cutaneous manifestations, such as heliotrope rash. Nailfold videocapillaroscopy (NVC) is a non-invasive method to investigate microcirculatory alterations. A frequently observed finding is the scleroderma-like (SL) pattern, characterized by marked morphological abnormalities, disruption of the vascular architecture, capillary loss, neoangiogenesis, and microhemorrhages. This pattern has been reported in about 70% of patients with DM and in 35% of those with ASyS.Material and Methods The aim of the study was to evaluate capillaroscopic features in patients with DM and ASyS, comparing the two groups and exploring potential associations with clinical and laboratory findings.Patients diagnosed with DM or ASyS were prospectively enrolled. Clinical and laboratory data were recorded, and NVC of the hands was performed. The evaluation included the number, distribution, and size of capillary loops, as well as the presence of microhemorrhages, avascular areas and signs of neoangiogenesis, with assignment of a general semi-quantitative score (0-3).Results A total of 50 patients were included (mean age 53 years; 36 females, 14 males). Of these, 30 had DM (60%) and 20 had ASyS (40%). tables 1 and 2 show the main clinical and serological characteristics of the patients. table 3 shows a comparison of the capillaroscopic parameters examined between patients with ASyS and DM. Compared with ASyS, DM patients showed a significantly higher frequency of the SL pattern (p=0.0223), avascular areas (p=0.0352), irregular capillary distribution (p=0.0259), and higher semi-quantitative scores overall (p=0.0345). Furthermore, the SL pattern was more frequent among ANA-positive patients (p=0.0407).In contrast, ASyS patients more often exhibited uncharacterizable NVC alterations (p=0.0223 ). Moreover, in this group the presence of giant capillaries was associated with higher CPK levels (p=0.0074 ). No correlations were found between capillaroscopic changes and organ involvement, myositis-specific or myositis associated antibodies.Conclusions The findings confirm that DM is more often associated with the SL pattern compared to ASyS, characterized by greater vascular disruption and more extensive avascular areas. Thus, NVC may be considered as a valuable tool in distinguishing between the two conditions, better defining their clinical phenotype.
