  "abstract": "Introduction Interstitial lung disease (ILD) is a frequent and severe manifestation in systemic sclerosis (SSc). The six-minute walking test (6MWT) is an inexpensive, readily available tool in clinical practice for the monitoring of exercise capacity in patients with SSc-ILD. It is however unclear whether reduced exercise capacity correlates with severity of lung involvement or with the underlying SSc more generally.Objectives To assess the impact of patient reported outcome measures (PROMs) and lung specific characteristics on exercise capacity assessed by the 6 minutes walking distance (6MWD) in patients with SSc-ILD.To compare the 6MWD in SSc-ILD patients compared to age- and gender-specific reference values (1).Material and Methods Methods: We followed 109 patients including 24 with SSc-ILD in a prospective cohort study at Oslo University Hospital. Patients were assessed at baseline and 52 weeks. Exercise capacity using the 6MWD was assessed together with forced vital capacity (FVC) and diffusing capacity of the lung for carbon monoxide (DLCO), respiratory symptoms assessed by the New York Heart Association (NYHA) functional class and Visual Analog Scale (VAS) for dyspnea and cough, and self-reported symptoms and disability by validated PROMs. All correlations were assessed using Spearman correlation. Changes in the score values were analyzed by Wilcoxon signed rank test. We calculated standardized references values of the 6MWD for each patient as in (1).Results Result: The study population included 24 patients with SSc-ILD, with the majority being female, mean age 66 years, and mean disease duration of 7.5 years ( table 1). We identified significant and strong correlations between lower DLCO and lower 6MWD at baseline and 52 weeks; and cough correlates moderately with lower 6MWD. At 52 weeks, we identified moderate correlation between NYHA >3 and fatigue with lower 6MWD. We did not identify any correlations between 6mwd and fvc, dyspnea vas, borg scale, or vas pain and patient global assessment (figure 1). The regression analysis identified an association between higher DLCO and longer 6MWD at 52 weeks. Interestingly, SSc-ILD patients had a significant shorter 6MWD compared to the calculated reference values (528m vs. 553 m, p=0.020).Conclusions Conclusion: Interestingly, low DLCO and 6MWD showed strong correlation at baseline and 52 weeks. There was no correlation with pain and only a moderate correlation with NYHA, chough, fatigue and ScleroID dyspnea. These findings suggest that the 6MWT is a reliable measure to assess the impact of ILD in SSc and supports the use in the monitoring of SSc-ILD.Abstract P.089 Figure 1Abstract P.089 Table 1Disease characteristics from baseline and 52 weeks",
