  "abstract": "Introduction PRDX-4 and PRDX-6 are members of a highly conserved peroxidase family involved in regulation of ischemia-reperfusion injury and cell proliferation, which are key events to the pathogenesis of SSc. The aim of this study was to investigated the role of PRDX-4 and PRDX-6 in the pathogenesis of SSc through evaluations of associations between their serum concentration and clinical features of SSc. Moreover, we evaluated the role of PRDX-4 and PRDX-6 in predicting progression of the disease.Material and Methods Serum concentration of PRDX-4 and PRDX-6 were measured by means of ELISA in 75 SSc patients and 20 healthy subjects. Progression of SS-related interstitial lung disease (SSc-ILD) was defined as >10% decline in FVC or >5% decline in FVC and >15% decline in DLCO. Progression of SSc was defined as death, development of a new organ complication (new onset ILD, pulmonary hypertension, primary heart involvement and/or scleroderma renal crisis) and/or progression of SSc-ILD defined as described above.Results The mean concentrations of both, PRDX4 and PRDX6, were elevated in SSc patients as compared with healthy controls (p<0.05 for both). Concentration of both PRDX4 and PRDX6 were higher in patients with diffuse SSc as compared with limited SSc (p<0.05 for both) and correlated with mRSS (PRDX4: r = 0,39, PRDX6: r=0,51, p<0,05 for both).We did not reveal significant difference in the concentrations of PRDX4 or PRDX6 between SSc patient with and without ILD or correlations with FVC or DLCO. However, SSc patients with subsequent progression of ILD had higher baseline concentration of PRDX4 and PRDX6 as compared with those without progression of SSc-ILD. In multivariate regression including other risk factor of progression of ILD (shorter disease duration, diffuse SSc) only PRDX6 showed significant association with subsequent progression of SSc-ILD.There were no significant associations between serum concentrations of PRDX4 or PRDX6 and presence of pulmonary hypertension, digital ulcers or laboratory markers of inflammation.However, SSc patients with subsequent disease progression had higher baseline concentration of PRDX4 and PRDX6 as compared with those with stable disease. In multivariate regression including another risk factor of progression of SSc (shorter disease duration) only PRDX4 showed significant association with disease progression.Conclusions Our results indicate that PRDX4 and PRDX6 may be involved in the pathogenesis of SSc. Moreover, we have shown that serum concentration of PRDX6 may be helpful in predicting progression of SSc-ILD while serum concentration of PRDX4 are associated with overall progression of SSc.",
