  "abstract": "Introduction Systemic sclerosis (SSc) is a connective tissue disease that can be characterized by unpredictable clinical course with the development of organ damage, functional impairments, changes in appearance, reducing quality of life, and disrupting work capacity. Disability and social deprivation due to SSc are widespread phenomena. Predicting work disability has significant scientific and practical importance for reducing the socio-economic burden of the disease. The aim of our study was to identify characteristics of SSc that most significantly influence loss of work ability.Material and Methods 60 patients with SSc were included: 50 women and 10 men (mean age 50.18 ± 9.86 years). The median duration of the disease from the onset of Raynaud’s phenomenon was 7.5 [3; 17.5] years, and from the first non-Raynaud symptom — 6.5 [2; 12.5] years. The severity of clinical, laboratory, and instrumental characteristics of SSc, as well as levels of anxiety and depression according to the Hospital Anxiety and Depression Scale (HADS), were assessed. Quality of life was evaluated using the Short Form-36 (SF-36) questionnaire, while functional impairment was measured using the Health Assessment Questionnaire (HAQ) and the Scleroderma Health Assessment Questionnaire (SHAQ). Dyspnea severity was assessed using the modified Medical Research Council (mMRC) scale. Possible predictors of disability were selected using binary logistic regression and further analyzed using multiple logistic regression.Results Using univariate analysis significant predictors of work ability loss were identified and integrated into a multifactorial model. These predictors included: HAQ and SHAQ scores, SF-36 Physical Component Summary score, HADS of depression, the level of dyspnea according to mMRC, systolic pulmonary artery pressure (SPAP) level, tricuspid regurgitation velocity, six-minute walk test, forced vital capacity (FVC), physically demanding occupations and Raynaud’s phenomenon, in combination with contractures, ulcers, dry necrosis or calcifications. Multifactorial analysis revealed that mMRC > 1 point (OR = 56.8, 95% CI 3.4-952.1, p=0.005), tricuspid regurgitation velocity > 2.4 m/s (OR = 113.5, 95% CI 8.1-1583.4, p<0.001) and Raynaud’s phenomenon, in combination with contractures, ulcers, dry necrosis or calcifications (OR = 14.3, 95% CI 1.4-151.5, p=0,027) were significant in predicting work disability in SSc. The ROC-curve is shown in Figure below.Conclusions The significance of assessing hand impairment, dyspnea severity using the mMRC scale, tricuspid regurgitation velocity by echocardiography in relation to predicting work disability underscores the necessity of evaluating these parameters in rheumatology practice.Abstract P.287 Figure 1",
