  "abstract": "Introduction Systemic sclerosis (SSc) is a chronic connective tissue disease characterized by inflammation and fibrosis, affecting multiple organs. Respiratory involvement, mainly pulmonary arterial hypertension (PAH) and interstitial lung disease (ILD), is associated with poor prognosis and is the leading cause of mortality in SSc. This study aimed to characterize the clinical features of SSc patients with ILD and to evaluate the treatment approaches.Material and Methods A total of 78 consecutive SSc- ILD patients were included in this cohort. Demographic characteristics, laboratory, pulmonary function tests and treatments were analyzed.Results There were 78 patients with SSc-ILD, the mean age was 49.89 ± 11.66 years, females made up 84.6%, mean disease duration was 14.38 ± 8.32 years. Mortality was 20.5%. Ana and antiscl-70 positivity was present in 93.3% and 73.1, respectively ( table 1).82.1% of SSc-ILD patients recieved corticosteroids, and 48.7% recieved antimalarials. Mycophenolate mofetil, rituximab and cyclophosphamide were the most commonly used immunosuppressive drugs (table 2).After treatment, the mean pulmonary function tests did not significantly differ from the baseline (p>0.05). A significant improvement was observed only in SPAP values (p: 0.028) (table 3).Conclusions In SSc patients with ILD, ANA and anti-Scl70 antibody positivity was frequent, and the most affected gender was female. Mortality remained high, and despite advances in diagnosis, treatment, and follow-up, optimal outcomes have yet to be achieved.
