  "abstract": "Introduction Anti-ribonucleoprotein (anti-RNP) antibody is associated with an increased cancer risk and poor survival in systemic sclerosis (SSc) patients. However, there are currently no data on the specificity of anti-RNP antibody among Thais, a population in which diffuse cutaneous SSc is more common than in Caucasians and some Asian populations. Clarifying these associations could improve prognosis and patient care.This study aims to investigate the prevalence of anti-RNP antibody, as well as the clinical features and outcomes of anti-RNP antibody positive patients, among Thais with SSc.Material and Methods A historical cohort study was conducted in adult SSc patients who were positive for anti-RNP antibody, and had at least one year of follow-up between December 1, 2014, and July 31, 2025. Patients with localized scleroderma or positive for anti-Sm/nRNP antibody were excluded. Prevalence of anti-RNP antibody, clinical characteristics, and mortality rate were analyzed.Results Anti-RNP was positive in 36 of 261 with the prevalence of 13.8% (95% CI 9.9-18.6). The majority were female (90%). The respective mean age at onset and mean disease duration was 51.5±15.8 years and 9.3±7.2 years. Among patients with anti-RNP positive, Raynaud’s phenomenon was revealed in 52.6%, esophageal involvement in 19.4%, intestinal involvement in 8.3%, and interstitial lung disease (ILD) in 50.0%. A total of 185 person-years of follow-up, four patients (11.1%) died resulting in a mortality rate of 2.16 per 100 person-years (95% CI, 0.81-5.85). The causes of death were pulmonary infection (2 patients), severe ILD (1 patient), and SSc-related event without specific organ involvement (1 patient). The median survival was greater than 10 years.Conclusions Anti-RNP-positive was uncommon among Thai patients with SSc. Approximately half of these patients developed Raynaud’s phenomenon and ILD, and the mortality rate was low. This suggests that the presence of anti-RNP may indicate a good prognosis among Thais SSc patients.",
