  "abstract": "Introduction Among systemic sclerosis (SSc) complications, heart involvement (SHI) – both primary (pHI) and secondary (sHI) – is often underdiagnosed. Data in the literature suggest that the prevalence of pHI may be higher than previously assumed. The clinical presentation of SHI may involve all cardiac structures. In recent years, the understanding and management of SSc-related pHI have improved. However, many potential risk factors for cardiac involvement in SSc remain unknown, particularly those based on additional tests such as the electrocardiogram (ECG) and laboratory markers (e.g., SSc-specific antibody profiles). The aim of this study was to evaluate the association between SSc–specific antibodies and clinical and subclinical SSc cardiac involvement.Material and Methods We collected complete clinical and laboratory data from 50 patients with SSc and scleromyositis, including laboratory parameters, ANA (antinuclear) SSc antibody profiles, imaging tests (such as echocardiography), and ECG findings. Within our multidisciplinary team (MDT), including cardiologist, we defined clear parameters to identify SSc- pHI. Cross-sectional analyses were performed to identify factors associated with SHI, including both pHI and sHI.Results Certain antibodies from the ANA SSc profile appeared to be associated with specific types of cardiac involvement. Relevant arrhythmias were observed significantly more often in individuals positive for PM-Scl-75 antibodies (42.86% vs. 6.98%; p=0.007) and Ku antibodies (60% vs. 6.67%; p<0.001). Moreover, RP155 positivity was significantly associated with a higher frequency of heart failure (HF) with mildly reduced ejection fraction (22.22% vs 2.86%; p=0.04), and PM-Scl-10 positivity was associated both with significant valvular disease (50% vs 8,7%; p=0.015) and right atrial enlargement on ECG 50% vs 4,35%; p=0.001). The presence of CENPB antibodies had a protective impact on heart disease (0% vs 21.21%; p=0.041). The presence of SCL-70 antibodies was numerically more frequent in patients with coronary artery disease and HF; however, this finding did not reach statistical significance (21.74% vs 3.85%; p=0,057 and 52.17% vs 25.93%; p=0.057 respectively).Conclusions In our study cardiac involvement in systemic sclerosis was associated with distinct autoantibody profiles. Antibody profiling may potentially support earlier detection and risk stratification of cardiac manifestations in systemic sclerosis, however larger studies are needed to confirm these findings and enable their application into clinical practice.",
