  "abstract": "Introduction Systemic sclerosis (SSc) is an autoimmune disease characterized by cutaneous and internal organ fibrosis, microvascular damage, and development of autoantibodies. Heart involvement is one of the most frequent manifestations and often determines prognosis.Within the spectrum of SSc lies prescleroderma (preSSc), which includes patients with Raynaud’s phenomenon, capillaroscopic abnormalities, and specific SSc autoantibodies. The factors influencing the progression from preSSc to SSc have not been established, nor have the long-term complications associated with these patients.Objectives: To identify subclinical heart involvement and relate these findings to the diagnosis of SSc and preSSc.To describe the different electrocardiographic patterns present in patients with SSc and preSSc.Material and Methods Observational, cross-sectional study of patients diagnosed with SSc (2013 ACR/EULAR criteria) and preSSc (LeRoy and Medsger criteria, 2001). Clinical, serological, and pulmonary function data, as well as concomitant diagnoses, were included. Heart involvement was evaluated through pro-BNP levels, high-sensitivity troponin T, and electrocardiogram (ECG). Each ECG was reviewed by a cardiologist, who classified the electrocardiographic abnormalities into 7 categories ( table 1):Results A total of 125 patients (89.6% women) with SSc and preSSc (n=75 and n=50, respectively), were included. Mean age was 59 years (range 50–69), age at diagnosis was 49 years (range 40–60), and Raynaud’s phenomenon was present in 94.55% of SSc patients and 96% of preSSc patients. Among SSc patients, 32.35% had diffuse forms, 63.24% limited forms, and 4.41% sine scleroderma.In the SSc group, 21.33% had another associated disease, most frequently Sjögren’s syndrome, followed by hypothyroidism. In the preSSc group, 12% had a secondary diagnosis, also most frequently Sjögren’s syndrome.The following table shows ECG data, pulmonary function, and capillaroscopy pattern (table 2):In SSc group, a significantly higher number of abnormal ECGs were observed compared to the preSSc group (p=0.031). A significantly higher percentage of pulmonary involvement was also noted in the SSc group (p=0.002).There were no statistically significant differences between the diagnosis of SSc and preSSc regarding the different ECG patterns or the pulmonary function test parameters.Conclusions Patients with SSc have a higher percentage of ECG abnormalities compared to the preSSc group, although no differences were observed among the different ECG patterns of involvement.Abstract P.158 Table 1Abstract P.158 Table 2",
