  "abstract": "Introduction Systemic sclerosis associated interstitial lung disease (SSc-ILD) is one of the most important manifestations determining morbiditiy and mortality in patients with SSc.Material and Methods A total of 388 SSc patients who were followed up between 1996-2023 and fulfilled the classification criteria for SSc(2013) were retrospectively screened; eighteen of the patients with ILD(n=190) were excluded due to insufficent data and finally 172 (157 females, 91.3%) SSc-ILD patients with a mean follow-up of 109.3±5.86 months included into the study.Results Demographics,clinical characteristics, and baseline respiratory parameters of patients with a mean ILD duration of 87.5±4.7 months are summarised in table 1. ILD was detected on baseline HRCT in 57.6% of patients. FVC<%80 was found in 84/137 of the SSc-ILD patients and DLCO<%80 was found in 98/128. Forty-three(25%) patients had smoke exposure.A total of 45 (26.3%, 36 females) SSc-ILD patients deceased after a mean follow-up of 225±12.7 months and an ILD duration of 185.7±11.8 months. Survival rates after diagnosis of ILD are 98.2% at 1 year, 96.2% at 2 years, 94.2% at 3 years, 88.9% at 5 years, 87.0% at 7 years and 68.9% at 10 years. Survival time was shorter in SSc-ILD patients who had male sex, an age of >45 at diagnosis, baseline DLCO<50% or ILD >10% on HRCT (81.6±10.7 vs 192.2±12.2, p=0.001; 134.9±8.6 vs 224.8±15.7, p=0.001; 140.8±22.3 vs 218.4±15.9, p=0.017; or 103.7±11.8 vs 199.6±8.5, p=0.003, respectively, figure 1) after diagnosis of ILD. Cox regression analysis of the parameters associated with survival time are summarised in table 2. Male sex (HR:4.3), an age of >45 at diagnosis(HR:2.7), and baseline DLCO<50% (HR:2.6) were factors associated with survival in univariate analysis. ILD >10% on baseline HRCT was a significant associated factor in univariate (HR:6.9) and multivariate (HR:10.5) analyzes. Survival time of SSc-ILD patients who developed ILD within the first 2 year of the diagnosis were shorter than those who developed after 2 years (177.2±15.9 vs 259.0±15.9, p=0.005).Conclusions Survival rates were found to be decreased to <90% 4 years after the onset of SSc-ILD. Male sex, diagnosis after age 45, baseline DLCO<50% or ILD >10% on HRCT were risk factors associated with mortality. Careful follow up and dynamic management should be applied to elderly and male SSc-ILD patients.
