  "abstract": "Introduction Increased lung cancer was reported in scleroderma. We aimed to characterize SSc patients with lung cancer (LuCa).Material and Methods Data on SSc patients from 2000 until 2025 (the 042 EUSTAR center) with follow-up above 1 year were extracted. LuCa were compared with non-LuCa-ILD (Ca-ILD, #13), non-LuCa non-ILD (Ca-SSc, #24) and SSc-ILD no cancer (#76). Student T-test, chi-squared test (p<0.05) were used.Results Seventeen out of 455 patients had LuCa (3.7%), 16 were analyzed. The age at the end of follow-up was not different between all cancer subgroups. SSc-ILD were younger then LuCa & Ca-SSc (p<0.036, p<0.009). At SSc diagnosis LuCa (44.0 [14.7]) and SSc-ILD (44.5 [13.0]) were younger then Ca-ILD and Ca-SSc (56.3 [15.9], 53.3 [14.1], p<0.002, p<0.04, p<0.025, p<0.003). The age at cancer diagnosis was not different (62.9 [14.3], 59.4 [16.2], 56.8 [15.7]). The time from SSc onset to cancer was longer in LuCa than in Ca-ILD and Ca-SSc (18.9 [10.6], 3.1 [7.1] and 5.9 [14.3]; p<0.0001, P<0.0009) with no difference between Ca-ILD and Ca-SSc. All LuCa developed cancer after SSc diagnosis, 8/13 Ca-ILD pts developed cancer after SSc, 19/24 Ca-SSc pts had malignancy before SSc. Patients characteristics presented in the table 1.Lung cancers data: 12 adenocarcinoma, 3 squamous cell carcinoma, 1 neuroendocrine cancer; 4/9 patients had positive molecular test (3-PD-L1 & 1-TP-53); 3 had pulmonary embolism, 3 malignant pleural effusion, 11 metastases; 8 underwent uneventful lobectomy (3 lived 8-14 years after); 9 chemotherapy, 1 radiation. Ca-ILD data: 4 breast, 5 GUT, 1 thyroid, 1 pancreas , 2 lymphoma. Ca-SSc data: 11 breast, 4 ovary, 3 GIT, 2 lymphoma, 2 thyroid, 1 larynx, 1 sarcoma; 5 pts had 2 or more malignancies. All cancer patients had higher mortality than SSc-ILD (p<0.014, p<0.002, p<0.001). Death occurred in 15 LuCa patients (93.8%, 11-cancer, 2- SSc), 7 Ca-ILD (53.9%, 3-cancer, 2-SSc), in 8 Ca-SSc (33.3%, 6-cancer, 1-SSc), in 22 SSc-ILD (28.9%, 20-SSc). Years from cancer diagnosis to the last follow-up: LuCa 3.3 [4.2], Ca-ILD 5.5 [4.7] and Ca-SSc 10.5 [10.5], (p<0.089, p<0.001).Conclusions SSc pts are at risk of LuCa at younger age, particularly late in SSc course. The risk profile includes diffuse subset, anti-topoisomerase antibodies, digital ulcers, and ILD. LuCa may be overlooked as severe SSc masks cancer symptoms and the performance of HRCT is less frequent. In the absence of malignant pleural effusion or metastasis the surgery may be favorable. In term of bad prognosis, the awareness on LuCa is crucial.Abstract P.416 Table 1",
