  "abstract": "Introduction The term Raynaud’s phenomenon (RP) is used to describe digital vascular compromise in response to cold exposure and emotional stress. The majority of affected people have no known cause and the term primary RP (PRP) is used. Low body mass index (BMI), migraine and fibromyalgia syndrome (FMS) are commonly identified amongst people otherwise classified as having PRP. We examined their impact on symptom burden in PRP.Material and Methods An online self-reported survey was made publicly available on the Scleroderma & Raynaud’s UK website (Nov 2024-Mar 2025), capturing RP features, demographics and relevant comorbidities. Impact of FMS and migraine on PRP symptom characteristics was assessed using chi-square and impact of the ordinal categories of BMI with the linear-by-linear association test.Results Among 4,141 respondents, 79.2% reported RP with no associated cause (designated PRP). BMI data were available for 3173 (96.8%) respondents with PRP with 15.2% classified as underweight. Self-reported FMS and migraine were present in 6% and 19% of PRP respondents respectively. Among respondents with PRP, a significant linear trend was observed with decreasing prevalence of cyanosis, hyperaemia, thumb involvement and triphasic RP symptoms, alongside increasing prevalence of tingling, as BMI category increased ( table 1). Sensory symptoms with RP pain and tingling (but not numbness) were more prevalent amongst PRP respondents with FMS. Patients with fibromyalgia reported less numbness. Concomitant migraine was associated with more prevalent cyanosis, hyperaemia, RP pain and thumb involvement.Conclusions BMI category, fibromyalgia, and migraine each influence the patient experience of RP symptoms in ‘primary’ RP. Lower BMI (and inherent thermoregulatory factors) is associated with an increased vasospastic burden. FMS is associated with greater sensory sensitisation and pain. The presence of migraine is associated with both RP pain and digital vasospastic features. Digital vascular symptom characteristics in ‘primary’ RP may be influenced by BMI and associated co-morbidities. Distinct aetiopathogenic factors may contribute to digital vascular symptoms in PRP and may warrant different approaches to classification and management.Abstract P.187 Table 1Impact of BMI category, fibromyalgia and migraine on RP symptom characteristics in primary Raynaud’s phenomenon",
