  "abstract": "Background While numerous procedural predictors of tremor outcome following MR-guided focused ultrasound (MRgFUS) have been identified, clinical factors remain understudied. Younger age is generally associated with better outcomes though increased cardiovascular comorbidities in older patients may confound this relationship. This study aimed to evaluate the influence of demographic factors, cardiovascular risk factors, and cerebral small vessel disease (CSVD) on tremor outcomes after MRgFUS.Methods In this single-centre study, sixty tremor patients were retrospectively analysed. Tremor severity was assessed at 1-year post-MRgFUS. Demographic data, tremor characteristics, cardiovascular risk factors, and CSVD data were collected.Results Active smoking increased the likelihood of experiencing a poor outcome following MRgFUS. Demographic factors, number of cardiovascular risk factors, and presence of CSVD did not significantly impact the likelihood of achieving a good tremor outcome.Discussion This project comprehensively explored the effect of clinical factors on post-MRgFUS outcomes. Previous literature points to acute and longer-term effects of nicotine on tremor severity, which may explain our findings. These findings suggest that smoking cessation may benefit patients undergoing MRgFUS. Patients should be advised to quit smoking prior to the procedure. A larger, multi-centre study is needed to confirm the effects of smoking and cardiovascular comorbidities on tremor outcomes.Kb720@ic.ac.uk",
