  "abstract": "Deep brain stimulation (DBS) is an established therapy for Parkinson’s disease (PD), yet individual variability in symptomatology underscores the need for personalized treatment strategies. The Medtronic Percept device facilitates real-time monitoring of local field potentials (LFPs) in the basal ganglia, providing novel insights into neural oscillations associated with motor symptoms. In this case series, we explored the dynamics of beta oscillations (13-35 Hz) in patients undergoing DBS and their correlation with clinical manifestations.Our cohort included three patients with distinct profiles. One patient exhibited worsening bradykinesia and freezing in the afternoon, corresponding to heightened beta activity. Another displayed speech difficulties aligned with afternoon peaks in beta power, while a third demonstrated morning-associated motor ”off” phases mirrored by increased beta amplitudes. These findings suggest circadian and patient-specific variability in beta oscillations, emphasizing their role in symptom modulation.Although our study highlights the potential of beta oscillation monitoring as a mechanism to drive adaptive DBS, limitations such as the lack of symptom diaries and the focus on motor symptoms alone limit broader applicability. Future studies integrating multimodal assessments could further refine personalized DBS strategies.
