  "abstract": "Background Migraine is the main cause of global disability in young people. Burden on inpatient services is not described. We aimed to identify (1) regional variations in migraine admissions and (2) differences in emergency and elective care.Method The Hospital Episode Statistics (HES) dataset includes episode-level anonymised-patient data of NHS hospital activity including hospital spells. We analysed admissions with a primary diagnosis of migraine between 2013-24, using odds ratios (OR) for crude counts, rate ratios (RR) and Poisson regression.Results There were 387079 migraine hospital spells between 2013-24. The mean age of those admitted was 42.1 years and 77% of admissions were female. The North-West had the highest total admissions (OR 3.343[3.290-3.397], p=0.000) but when accounting for regional population the North-East was higher (RR 1.454[1.381-1.531], p=0.000). Emergency admission numbers were highest for the North-West (OR 3.910[3.394-3.869], p=0.000) but the population rate was higher in the North-East (RR 2.992[2.821-3.173], p=0.000). Elective admission numbers were highest for London (OR 25.63[19.16-35.29], p=0.000) as was the population rate (RR 5.928[5.369-6.564], p=0.000.Conclusions There is significant variation in migraine inpatient spells regionally with a North-South divide in terms of emergency and elective admissions.
