  "abstract": "Background Traumatic brain injury (TBI) is often reported to be a risk factor for the development of neuropsychiatric problems, however the relationship between TBI severity and subsequent neuropsychiatric outcomes remains unclear. Investigating this issue has been difficult due to a lack of representative, longitudinal data on TBI that includes multiple adequate measures of injury severity, mental health symptoms and service use.Methods This study utilised data from the TRACK-TBI longitudinal study, which included 2,700 participants with TBI recruited from 18 level 1 trauma centres across the United States. The association between over 10 measures of TBI severity at hospital admission and psychiatric symptom scores at 12 months post-injury, as well as psychiatric service utilisation during the first year post-injury, were examined. Mixed-effects models were used and adjusted for demographic and social covariates, with missing data imputed using random forest multiple imputation.Results TBI severity measures, including the Glasgow Coma Scale, Marshall score, duration of loss of consciousness, post-traumatic amnesia and alteration of consciousness and presence of abnormalities on computed tomography imaging, were all positively associated with higher mental health symptom scores, measured psychometrically using BSI-18 and PHQ-9 total scores, and increased psychiatric service use, including psychotherapy and psychiatric medication use. These associations remained after adjustment for a range of potential confounders.Conclusion This study presents evidence supporting a dose-response relationship between the severity of TBI and subsequent psychiatric symptoms and service utilisation in first year following injury, and emphasises the importance of longitudinal research in this area.",
