  "abstract": "Background Lung cancer is the most common and deadly type of malignant tumor worldwide, the project aims to compare the real-world effectiveness and safety of first-line PD-1 inhibitor plus chemotherapy versus bevacizumab plus chemotherapy in advanced driver gene-negative non-squamous non-small cell lung cancer (nsNSCLC).Methods We conducted a retrospective study of patients with advanced driver gene-negative nsNSCLC receiving first-line therapy with either PD-1 inhibitor plus chemotherapy (IC group) or bevacizumab plus chemotherapy (BC group). Study endpoints included progression-free survival (PFS), overall survival (OS), objective response rate (ORR), disease control rate (DCR), and treatment-related adverse events (AEs). Propensity score matching (PSM) was used to adjust for baseline covariates ( table 1).Results Among 228 enrolled patients, the IC group (n=116) demonstrated significantly longer median PFS compared to the BC group (n=112) (12.97 vs. 8.63 months; HR [if available]; P=0.002). Median OS was numerically longer in the IC group (35.27 vs. 29.87 months) but did not reach statistical significance (P=0.061). No significant differences were observed in ORR (P=0.195) or DCR (P=1.000) ( figure 1). Treatment-related AEs occurred in 54.31% (63/116) of IC patients and 52.68% (59/112) of BC patients (P=0.415). After PSM (43 patients per group), median PFS was 11.20 months (IC) versus 7.87 months (BC), while median OS was 29.60 months (IC) versus 23.77 months (BC). These differences were not statistically significant (figure 1). AE incidence remained comparable between groups (P=0.829).Conclusions In this real-world cohort of advanced driver gene-negative nsNSCLC patients, first-line PD-1 inhibitor plus chemotherapy demonstrated significantly improved PFS compared to bevacizumab plus chemotherapy in the primary analysis. While OS favored the IC group, this difference was not statistically significant.
