  "abstract": "Background Peer-led practice groups are increasingly used in public health to reinforce behaviour change, promote social connectedness, and improve intervention sustainability. Among older adults, such groups offer potential to support wellbeing, reduce isolation, and translate therapeutic strategies into everyday practice. However, barriers to engagement remain poorly understood. This qualitative study explores older adults’ experiences of participating in online peer support groups following structured therapeutic programmes, with a focus on identifying actionable insights for improving uptake, adherence, and impact.Methods A service evaluation was conducted using individual interviews (n=10) and focus groups (n=12) with older adults who had participated in recent wellbeing or therapeutic programmes delivered online. Data were analysed thematically to identify key factors affecting engagement and perceived value of peer-led group formats.Results Four interrelated themes emerged as critical to sustained engagement: (1) perceived psychological safety within the group; (2) uncertainty about group purpose or structure; (3) the importance of clear, timely communication; and (4) the value of practical guidance and facilitation. Ambiguity around the format, expectations, or relevance of the group often reduced initial attendance and ongoing participation. Flexibility in scheduling and responsiveness to individual needs also enhanced adherence.Conclusion To move from research to reality, the design and implementation of peer support spaces should explicitly address communication, clarity, and psychological safety. Embedding these practical insights into public health programme design could improve the accessibility, effectiveness, and sustainability of group-based interventions for older adults—particularly in digitally delivered formats.",
