  "abstract": "Background Early diagnosis of mild cognitive impairment (MCI) or dementia could facilitate access to support and care planning. The UK National Screening Committee currently recommends against screening in people without previous suspicion of cognitive decline. We performed a mapping review to assess the volume and type of recent evidence on the accuracy of screening tests for MCI and/or any type of dementia.Methods We searched five bibliographic databases for studies published between January 2018 and June 2024. Eligible studies assessed the accuracy of any type of screening test in individuals without previous suspicion of cognitive decline. Studies involving individuals with co-morbidities that could affect cognitive performance were excluded. We prioritised diagnostic cohort studies and systematic reviews of such studies. We extracted descriptive data from included studies and grouped studies by design for narrative synthesis.Results We included 53 studies, comprising 20 systematic reviews, 14 diagnostic cohort studies, 12 diagnostic case-control and 7 observational studies. Systematic reviews evaluated the accuracy of single or multiple brief cognitive tests in primary care and/or community settings. The most comprehensive systematic review included 59 studies assessing 49 different screening tools. This review found moderate certainty evidence of brief screening tools demonstrating adequate sensitivity and specificity for detecting dementia, whereas the evidence for MCI was inconsistent and imprecise. A variety of other approaches to screening were covered by one or more systematic reviews. Beyond the scope of the mapping review, we identified a randomised trial comparing screening with no screening. The study found no differences in anxiety or depression at 1 month or health-related quality of life at 12 months.Conclusion Despite the publication of various new and/or updated systematic reviews, there is no consensus on the choice of tools for population screening for MCI or dementia. While a more detailed evidence synthesis may help to clarify the issue, further research on the benefits and harms of screening is also required.",
