  "abstract": "Background Trauma registries are essential for improving trauma care and informing injury prevention programmes, yet their sustainability remains a challenge in low and middle-income countries due to financial constraints. This study estimated the cost of establishing and maintaining a feasible trauma registry in a tertiary hospital setting in Nigeria, addressing the scarcity of costing studies for African trauma registries and supporting cost-effective investments in trauma care systems.Methods This study utilised a previously developed prototype trauma registry for University College Hospital, Ibadan, Nigeria. The registry comprises 23 cost components, which were further classified into fixed and variable costs. Cost data were obtained by averaging multiple sources when relevant or selecting the most context-appropriate data. The Time-Driven Activity-Based Costing method was used to estimate start-up and annual maintenance costs. A deterministic sensitivity analysis was conducted to identify the most sensitive cost parameters, with registry input parameters variably adjusted by ±20%.Results The estimated cost of establishing a trauma registry in University College Hospital, Ibadan, Nigeria, was US$18 189, with an annual maintenance cost of US$25 606. According to the deterministic sensitivity analysis, the estimated start-up and annual maintenance costs were most sensitive to changes in the cost of training healthcare workers and data extractor officers, respectively.Conclusions The study identifies key cost drivers and provides specific cost data to support stakeholders in planning and scaling trauma registries in Nigeria and resource-constrained settings, while also aiding the development of long-term funding strategies.",
