  "abstract": "Background Radiation dose in coronary CT angiography (CTCA) is a key safety metric. Advances in CT hardware and software enable substantial dose reductions while preserving image quality.Aim To evaluated the impact of evolving CT technology on CTCA radiation dose and image quality at a three-site UK centre (HFFT) over 6 years, 2019-2025.Methods  We retrospectively analysed audit-data on CTCA, collected from 2019 to 2025 by one operator on four scanners (Siemens Flash, GE HD750, Canon Prime, Canon Prism). We recorded dose-length product (DLP, mGy·cm) and the operator’s image-quality score (0-10). We compared Prism (2024-25) with earlier systems, including pairwise (Prism vs Prime in 2024), and regression analysis; multivariable linear regression (outcome=DLP, predictors=scanner type + year) adjusted for time trends; a similar model assessed quality.Results 1567 scans (DLP) and 1362 quality scores were analysed. Mean DLP was lowest for Prism (158.9 ) versus Prime (271.2) and Flash (187.6). In 2024, median DLP was 119 mGy·cm (Prism) vs 237 mGy·cm (Prime) (p<0.001), and Prism images had slightly higher mean quality (7.38 vs 6.80, p=0.0002). Adjusted regression showed Prism was independently associated with a large DLP reduction. Scanner type explained ~10% of DLP variance (R2≈0.10). Prism also yielded significantly higher quality scores (+0.62 vs Prime, p<0.0001) (table 1, figure 1).Conclusion The newer 16 cm-detector Canon Prism scanner delivered substantially lower radiation dose than older platforms while preserving or modestly improving image quality. This real-world trend over time supports technology-driven dose reduction in CTCA. Compared to older CT scanners, the Canon PRISM scanner in this study was associated with:A.
