  "abstract": "Successful awake paediatric cardiac MRI (CMR), relies on child’s cooperation during prolonged, noisy image acquisition & benefits from effective anxiety reduction strategies. Commercial MRI-compatible audiovisual distraction systems improve compliance & reduce sedation/general anaesthetic need. But their prohibitive cost limits routine NHS adoption. We designed &successfully implemented low-cost audiovisual solution using existing scanner infrastructure and non-ferromagnetic components.A bespoke system was developed for our 1.5 Tesla Siemens Sola MRI environment using a high-lumen consumer projector from the control room. Images were projected through through a Faraday-shielded observation window onto a rear-projection acrylic screen placed at the patient’s foot end within the scanner room. The child viewed the images via standard MRI lookout mirror arrangement, while audio was transmitted through the Siemens intercom pathway to pneumatic MRI-compatible headphones. The design avoided powered electronic devices into magnet room & preserved radiographer sightlines. Practical evaluation focussed on feasibility, image visibility, workflow integration, safety, and impact on successful completion of awake paediatric scans.The system was successfully implemented at a total equipment cost of approximately £1600, compared with >£30,000 for commercial systems. It integrated smoothly into routine workflow & provided clear visual and audio quality. No MRI safety incidents or image artefacts attributable to the system were observed. The setup improved tolerability of awake paediatric MRI, facilitating scans that might otherwise have required sedation/general anaesthesia.
