  "abstract": "Background Hypertrophic cardiomyopathy (HCM) is the most common inherited cardiac condition encountered in clinical practice, and until recently, was considered the leading cause of exercise related sudden cardiac death in young individuals. Historical exercise recommendations restricted individuals with HCM to low dynamic and low static physical activity. Currently, over half of these individuals fail to meet recommended levels of physical activity. A sedentary lifestyle promotes obesity, increases cardiovascular risk, adversely affects mental health and wellbeing. This is compounded by the lack of high-quality evidence supporting exercise recommendations, variation in clinical phenotypes within the same condition and sparse physician education around exercise prescription.However, recent evidence has suggested certain individuals with HCM may be able to engage in higher intensities of exercise. Improvements in the identification of individuals with HCM through cardiac screening, together with advances in medical therapies and interventions has resulted in a growing population of individuals with HCM who are keen to exercise. The British Association for Cardiovascular Prevention and Rehabilitation (BACPR) standards outline core components for cardiac rehabilitation. Existing guidance is largely oriented towards conditions such as coronary artery disease and heart failure. However, there is a lack of disease-specific guidance for individuals with HCM.Moving Medicine, developed by the Faculty of Sport and Exercise Medicine in partnership with Public Health England and Sport England, was officially launched by the Secretary of State for Health and Social Care at the 7th International Society for Physical Activity and Health Congress in London (2018). This online resource was developed following a knowledge into action framework (https://movingmedicine.ac.uk/consultation-guides/evidence-review) to assist physical activity conversations between healthcare professionals and their patients, by adopting a motivational interviewing framework, and to provide evidence-based recommendations. In response to the unmet needs of HCM patients we developed a disease-specific Moving Medicine module to provide structured, evidence-based guidance on physical activity for both healthcare professionals and patients.Methods A quantitative review was conducted following a literature search using PubMed, Web of Science and TRIP, and a qualitative review utilised PubMed and Web of Science. A scoping review was conducted in order to address the following questions and the evidence was graded:What are the benefits of moderate and high intensity physical activity in patients with HCM?What are the adverse events/risks of moderate and high intensity physical activity in patients with HCM?What are the effects of physical activity on cardiovascular remodelling in patients with HCM?What is the evidence on exercising with implantable cardioverter defibrillators in patients with HCM?What are the effects of physical activity in genotype positive phenotype negative individuals?A qualitative evidence review focused on barriers, facilitators and motivators to physical activity in both healthcare professionals and clinicians. A subsequent focus group brought together key stakeholders such as clinicians, cardiac rehabilitation experts, physiotherapists, specialist nurses, genetic counsellors, patient and charity (Cardiomyopathy UK, Cardiac Risk in the Young) representatives. This workshop facilitated the development of module content, including outlining the benefits of exercise in HCM, creation of readiness cycles, reviewing barriers to exercise and providing recommendations, addressing common patient concerns (figure 1) and creation of ‘top tips’ for a patient information sheet (figure 2).Conclusion The HCM Moving Medicine Module draft website was reviewed and went live in May 2020, offering healthcare professionals and patients concise evidence-based information and a clear framework for effective conversations that support individuals with HCM to incorporate physical activity into everyday life. Future work will focus on reviewing the analytics to understand user experience and preferences, as well as adapting this structured approach for other inherited cardiac conditions.Abstract 140 Figure 1Abstract 140 Figure 2",
