  "abstract": "A 54-year-old woman presented with progressive exertional dyspnoea two decades after surgical closure of an atrial septal defect (ASD). Transthoracic echocardiography demonstrated moderate tricuspid regurgitation, pulmonary hypertension with an estimated systolic pulmonary artery pressure of 68 mm Hg and an apparent small right-to-left shunt across the interatrial septum ( figure 1A, online supplemental video 1). A thin membrane-like structure was also noted within the left atrium (figure 1B). Electrocardiography demonstrated atrial tachycardia (figure 1C). Given the discrepancy between the minimal shunt and the severity of pulmonary hypertension, further investigation was pursued.",
