  "abstract": "A male patient in his 60s was admitted to our hospital with a complaint of dyspnoea. The dyspnoea was aggravated by physical activity and exhibited nocturnal exacerbation. His blood pressure was 140/70 mm Hg, with no history of myocardial infarction, and physical examination and ECG showed no significant abnormalities. Laboratory tests revealed elevated C-reactive protein (38.90 mg/L), erythrocyte sedimentation rate (44 mm/hour) and serum immunoglobulin G4 (IgG4) (18.30 g/L).",
