  "abstract": "An asymptomatic 52-year-old female was incidentally found to have cardiac enlargement. Her bilateral limb oxygen saturation levels were 99% and 97%, respectively. Subsequently, a Holter revealed sinus bradycardia (average 48 beats/min) and a long pause with the longest RR interval of 2.8 s. Transthoracic echocardiography showed right cardiac enlargement, mild pulmonary hypertension and abnormal widening of inferior vena cava (IVC). However, the atrial septum is intact via the parasternal four-chamber view ( figure 1A). An abnormal blood flow was detected in the right atrium (RA), directed toward the IVC (figure 1B). Contrast echocardiography revealed non-persistent saline contrast microbubbles in the lower left of left atrium (LA) (figure 1 C-D, shown by the red triangle) after three cardiac cycles following right heart cineangiography, appearing faster and denser via the IVC pathway (figure 1C, online supplementary S1) than via the superior vena cava (figure 1D, online supplementary S2).",
