  "abstract": "Background Anthracycline-based chemotherapy, although effective in treating breast cancer, is linked to cardiovascular toxicity and functional decline. While vigorous exercise may offer cardioprotective benefits, its feasibility in hospital-based supervised formats during chemotherapy remains under-explored.Aim To assess the feasibility of delivering a supervised, hospital-based vigorous aerobic interval plus resistance exercise intervention during chemotherapy, and its potential effects on cardiovascular function, physical outcomes, and quality of life.Methods This single-centre, two-arm randomised controlled feasibility trial will recruit women (n = 30) with breast cancer undergoing an 18-week anthracycline-based chemotherapy regimen. Participants will be randomised to receive either usual care or usual care plus a supervised exercise intervention ( tables 1 and 2) delivered during treatment. Feasibility will be evaluated through recruitment and retention rates, adherence, intervention fidelity, and patient-reported outcomes. Cardiovascular outcomes will include changes in electrocardiographic (ECG) activity, endothelial function (flow-mediated dilation), arterial stiffness (pulse wave velocity), and biomarkers of cardiotoxicity (troponin T and NT-proBNP). Additional assessments include functional capacity (6-minute walk test), muscular strength (handgrip dynamometry, 30-second sit-to-stand), self-reported physical activity (7-day Physical Activity Recall), and health-related quality of life (FACT-B, FACIT-F). Ethical approval has been granted for recruitment at the National Health Service site. Initial baseline data show comparable study groups. Planned analyses include ANCOVA, using baseline values as covariates to assess between-group differences over time. Qualitative data from participant questionnaires will be coded and thematically analysed using NVivo software.Conclusion This study will provide essential insights into the feasibility and potential cardioprotective benefits of implementing a structured, vigorous exercise programme during chemotherapy. If feasible, it may offer evidence to support the integration of exercise into standard oncology pathways to reduce cardiovascular risk, maintain physical function, and improve survivorship.",
