  "abstract": "Introduction Technicalities and associated costs limit the measurement of insulin resistance (IR) (linked to atherosclerotic cardiovascular disease) in routine clinical practice. The Triglyceride Glucose (TyG) index has been demonstrated to be a highly sensitive marker of IR (1,2). However, its relationship with a 10-year cardiovascular risk (CVD) in hypertensives in Nigeria is currently unknown.Methods This was a 15-year retrospective cross-sectional study involving the medical records of 1,430 adult Nigerians with hypertension. The TyG index of study participants was divided into quartiles, and the TyG index of study participants was calculated to explore the association with 10-year CVD risk. The Framingham 10-year Cardiovascular Risk Score was used to estimate the 10-year CV risk of the study population.Results The mean age of the study population was 55.8±13.9 years with a male preponderance of 54.1%. The mean TyG index was 8.72±0.6 with a significantly higher value in males (8.77±0.6 vs 8.67±0.5, p=0.001), this trend is consistent across all age groups. Traditional cardiovascular risk factors such as blood pressure, total and low-density lipoprotein cholesterol, glycated haemoglobin, uric acid and triglyceride increased across the quartiles, with Q3 and Q4 having higher values. After adjusting for well-known risk factors, an elevated TyG index was associated with an increased 10-year CVD risk in females only.Conclusion The TyG index is associated with an increased 10-year cardiovascular risk, and thus, elevated serum triglyceride and glucose levels, especially among hypertensives, must be screened and treated.References Park HM, Lee HS, Lee YJ, Lee JH.
