  "abstract": "Background Disparities in the cardiovascular outcomes between men and women have been widely published. The rapid access chest pain clinic (RACPC) is the national pathway for the diagnosis and initial management of ischaemic heart disease in the UK. Were such a pathway to introduce sex-based disparities then this might contribute to longer term inequalities.Methods A single-centre retrospective review of concurrent patient referrals to the RACPC between Jan-March 2019, with a five year follow-up period was undertaken. Sex-based differences in 1) discharge rates without investigation, 2) first-line diagnostic modality, 3) revascularisation referral rates, 4) adherence to guideline directed medical therapy (GDMT), 5) second line testing and 6) re-referral rates to secondary care were compared.Results 304 patients were included, average age 62 ±12 years, 58% male. There were no disparities for discharges without investigation between men and women, OR 0.88 (95%CI: 0.4–1.9, P= .7), initial anatomic versus functional diagnostic strategy, OR 0.7 (95%CI: 0.4–1.4, P= .3) or revascularisation referral rates, OR 3 (95%CI: 0.7–14, P= .2). Where indicated men were more likely to receive GDMT, OR 2.3 (95%CI: 1.2–4.4, P< .05) and high intensity statins, OR 3.4 (95%CI: 1.7–6.7, P< .05). For patients with negative initial investigations men were more likely to be referred back to the service, OR 3.2 (95%CI: 1.1–9.3, P= .04) despite similar re-presentation rates.Conclusions There was no disparity in the investigation of chest pain between men and women in our RACPC service. However, men were more likely to receive GDMT and be re-referred back to the service during follow-up.",
