  "abstract": "Background Home blood pressure monitoring (HBPM) is a well-established method for managing hypertension in primary care, yet patient adherence remains a challenge. We introduced a quality improvement programme across four GP practices in England to evaluate the impact of structured digital engagement strategies to improve HBPM.Methods This retrospective cohort study analysed the records of 196 hypertensive patients from a primary care practice over a 24-month period. In January 2023, the practice implemented a co-designed patient engagement strategy for all hypertensive patients(n=1108), including:Automated reminders through text and email at regular intervals.A colour-coded spreadsheet (figure 1) to facilitate feedback on blood pressure control.Personalised feedback optimise management, including medication reviews to investigate the need for up titration or addition of anti-hypertensive medication.Access to BP monitors provided by the practice, to be borrowed and returned, if needed.In this report, we present BP submissions and control rates for a random sample of 196 out of 1108 patients, comparing baseline data from 2022 (pre-intervention) with post-intervention data from 2023 and 2024.Abstract 7-034 Figure 1Guide for self-monitoring spreadsheet sent to hypertensive patientsResults Following implementation of the engagement strategy, both HBPM participation and BP control improved. ( figure 2):Baseline (2022, Pre-Intervention): 119/196 (60.7%) patients submitted BP readings; 62 (31.6%) achieved target BP.Year 1 (2023, post-intervention year 1): 138/196 (70.4%) submitted readings (+16% from baseline); 81 (41.3%) achieved target BP.Year 2 (2024, post intervention year 2): 159/196 (81.1%) submitted readings; 126 (64.2%) achieved target BP (+30.6% from baseline).36(19 males, 17 females) of the 196 patients failed to send any home BP readings despite measures taken for patient engagement. Patients were found to be more unlikely to submit home BP readings if over the age of 65 (67%).Abstract 7-034 Figure 2Impact of patient engagement strategies on HBPM and BP controlConclusion Structured digital engagement strategies, including timely reminders, self-monitoring tools, and personalised feedback, significantly improved HBPM participation, especially in younger hypertensive patients (<65 years). Patients who engaged with these interventions were more likely to achieve target BP, demonstrating the effectiveness of proactive digital strategies in hypertension management. The percentages of HBPM and BP control showed incremental improvement over the two years of intervention. This trend highlights the possibility of such digital interventions allowing improved control of blood pressures over time, as patients continue to familiarise themselves with these methods and become empowered to understand their hypertensive journey and management. Our study supports the integration of patient-centered digital interventions into routine hypertension care to improve long-term cardiovascular outcomes.",
