  "abstract": "Introduction The NHS Long Term Plan identifies cardiovascular disease (CVD) as the biggest primary area where lives can be saved. Primary care plays a significant role in the management of patients at risk of CVD. Through a fellowship education programme, clinicians may be supported to improve confidence and management of high-risk conditions.Methods Across 3 cohorts, a series of monthly clinical webinars over 7-months, each year, was developed for 192 healthcare professionals in South London between April 2022 and November 2024. Topics included: Atrial Fibrillation, Lipid Management, Chronic Kidney Disease, Diabetes, Heart Failure, Behaviour Change and Hypertension. Alongside clinical training, the fellows undertook an improvement project which used quality improvement (QI) skills developed as part of the programme, to address an area of need within their practice or PCN. Results In total there were 40 educational sessions, of these, 22 were clinical webinars and 16 were QI sessions. In total, at least 13 hours of education was delivered to each fellow. Across the 3 years of the programme, 118 Fellows completed final reports on a total of 98 improvement projects. The Fellows reported the number of patients their projects impacted in various ways, by combining each reported project the combined impact is estimated to be:5,239 patients were contacted.1,374 patients undertook some form of testing .766 patients had their coding updated. 1,071 patients received new or updated treatments. 107 patients were referred to secondary care. A survey conducted at the end of each cohort, with feedback from 95 Fellows revealed that as a result of the fellowship: 97% agree they are more confident in delivering care to patients at risk of cardiometabolic disease.85% agree that their practice/PCN has improved the way it manages patients at risk of cardiometabolic disease.91% stated their confidence in undertaking QI projects has increased. 96% agree that they are supporting their colleagues more with CVD care.Conclusions/Implications The fellowship programme was well-received, with participants finding it highly relevant to patient care. Given that many high-risk cardiometabolic conditions, such as hypertension and hyperlipidemia, are often asymptomatic, early identification and intervention are essential for improving long-term outcomes. Through structured education and QI-driven initiatives, the programme helped bridge the gap between clinical guidelines and real-world practice.Primary care operates in a complex and high-pressure environment, where competing demands can hinder the implementation of best practices. This fellowship demonstrated that targeted, practical education—coupled with hands-on QI support—can drive meaningful improvements in cardiometabolic disease management. While challenges remain in sustaining long-term change, this model offers a scalable approach to upskilling clinicians and enhancing patient outcomes.",
