  "abstract": "Introduction Hypertensive heart disease (HHD) is associated with significant morbidity and mortality with several nations witnessing a significant increase in the incidence and mortality of HHD. Accordingly, it is of significance to evaluate the temporal trends in the mortality of HHD across the United Kingdom (UK) to enable effective risk stratification and implementation of health policy interventions in addition to tailored management.Methods Temporal trends in the age-standardized mortality rate (ASMR) of HHD across the UK, and across the 4 nations (England, Northern Ireland, Wales, and Scotland) over the period 1990–2019 were evaluated by initially retrieving data from the Global Burden of Disease database. Joinpoint analysis software was utilized to calculate the Annual Percent Change (APC) and the Average Annual Percent Change (AAPC).Results Over the period 1990–2019, an estimated total of 109,477 HHD-related deaths with a female predominance of 57.6% were reported across the UK. A statistically significant increase in the ASMR of HHD was noted across the UK with an AAPC of 0.42 (95%CI 0.34 to 0.50, p<0.001). Further analysis revealed statistically significant increase in the ASMR in England and Northern Ireland with the latter witnessing a more prominent incline (AAPC 1.50, 95%CI 1.07 to 1.93, p<0.001). In contrast, Scotland and Wales observed statistically insignificant increases in the ASMR. Across England, a statistically significant increase in the ASMR was noted across all the regions except for Greater London where a statistically significant decline in the ASMR was observed (AAPC -0.13, 95%CI -0.27 to -0.001, p=0.049). Among the other regions, Yorkshire and the Humber encountered the highest incline in the ASMR with an AAPC of 0.77 (95%CI 0.66 to 0.87, p<0.001), followed by the East Midlands (AAPC 0.70, 95%CI 0.59 to 0.79, p<0.001) and South West England (AAPC 0.67, 95%CI 0.56 to 0.79, p<0.001).Conclusion Overall, a statistically significant increase in the HHD-related mortality was observed across the UK and the 4 nations. However, Greater London was found to encounter a statistically significant decline in the mortality rate. Therefore, prompt recognition of these findings is warranted to determine the factors leading to these trends.",
